Because that means "There exist one or more doctors such that I need them."
Even if we pin down the number using "pa mikce" then there is no implication
that "any doctor will do", but rather that one doctor is (and the rest
may not be) suitable for my need.
Therefore, as so often in Lojban, we should instead say what we mean:
mi nitcu lo nu (pa)da mikce mi
Or if instead we need a doctor for some other purpose, then:
mi nitcu lo nu (pa)da poi mikce cu batci mi le zargu
or whatever.
