Me too.  Our excuses are two-fold. 1) we got hit with a whole lot of stuff all at once (or over a relatively short time) and 2) many of these problems only become apparent when you try to do something with the whole -- whether use it or analyze it.  We were only asked whether this came up to what had been discussed and decided.
