I don't ever get to see "kch" in Swiss German spelling.  We
usually write /kX/ as |k|, while /k/ mostly comes from |gg|
(Nuggi /'nukki/ "pacifier") or foreign |c, qu| etc (Coggi 
/'kokki/ "coke", Quiche Lorraine /kiS 'lor&:n/).  ;o)  The
/X/ by itself is always written |ch|, no matter the place
in the word (Chuchichästli /'XuXi,XaStli/ "kitchen cupboard").

(Note that all Swiss German spelling is subjective and 
unofficial, except for the Bärndütsch dialect, which has an
official spelling.)

For no obvious reasons.  I can't think of a language where 
/x/ -> |ch|, but not initially.  High German just doesn't have
any native words starting in /x/.  Though I think ['Ci:na] ~
/'xi:na/ is a legal pronunciation for China, for example.  Hm.
